Every Maxicom Certificate of Destruction is a single document covering one job. Sections, in order: header (job ref, customer ref), pickup details (date, location, hand-over individual), destruction details (date, location, two operators, witness), per-asset table (one row per device), wipe-log or shred-batch-ID per device, signatures, standards citation, alignment statements, downstream-recipient log.
Sample uses the Maxicom format MXSG-2026-A-04217 for the job reference (A = sell-to-us; B = service-request; C = buy-from-us). Customer reference is your PO or contract number — appears alongside the job ref.
The per-asset table shows columns: serial number, make, model, capacity, classification (Low / Moderate / High), method (Clear / Purge / Destroy), completion timestamp, operator ID. For Clear / Purge, an additional wipe-log entry shows pass count and operator. For Destroy, the shred batch ID with photo-evidence reference.
Standards citation block names NIST SP 800-88 Rev. 1 + IEEE 2883-2022 explicitly, plus DoD 5220.22-M when used. Alignment statements follow: PDPA Section 24 statement is on every Certificate; MAS TRM-aware citation for FI customers; CCoP 2.0 citation for CII customers; ESG / Scope-3 appendix on request.
